This has got to be the best place to send a loved one or to be a patient in here. The last place I was in was absolutely horrible compared to this place! The nurses Minnie (always above and beyond), Sarah-fina ( love you, ) , Nadia (absolutely beautiful and kind ) Michelle ( always a smile and engaging) amazing crew!!! I know I’ve forgotten many names on this for you guys. My apologies. The CNA’s… Oh my goodness, cannot say enough positive about all of them. Maria LS I believe – amazing woman! Always there to help always has a smile and a good friend. Maria S -gosh I hope I’m getting this right -(you guys know ) always sunshine for every room, very helpful very warm very accepting. Rosie- very engaging and very helpful. Always a smile brought to all of us.. Autumn and Maryah also very sweet. I know I’ve missed several of you. I am so sorry and I didn’t mean to leave you out, but you guys are also amazing. Swingshift crew. Antonette what an amazing lady you are! Thank you for all of our talks all of our laughs. Oh my goodness lol almost getting too loud. Your daughter is wonderful too, and I forgot to mention her. She is an amazing nurse. Full of smiles, always polite and respectful.. Alex, I miss you! You were so much fun and so kind and silly. You’re very funny. And I know there’s some that I forgotten there in the graveyard crew I’m sorry I didn’t get your names, but you guys were just as amazing. Physical and occupational therapy – Boone – good morning! Always very kind, Sarah, thank you for your patience. Mike everyone “loves”🎾 you, keep on with the smiles. Kameron I enjoyed our talks. I enjoyed our time together thank you. The activity director is amazing Paula you do such great activities with everyone and you treat everyone so respectfully your calendars are amazing! Ro, kitchen manager- what great foods you have! You have always been great in the kitchen we’ve known each other 15 years plus and I hope we have many more years to come. There’s so many that I have not named in housekeeping in administration, but just know if you wanna come here? This is the absolute best place in the world. You will always be remembered and I will always miss you. I kind of hope I get to come back lol

FGPA- I cannot say enough about this place with such positiveness! I was immediately put to ease by the staff here- I had a bad experience at another rehabilitation center and was so afraid to be treated that way again but I was immediately put to ease by the everyone , they are excellent here. They treated me with such kindness and respect plus they were right there for me when I needed them. They never made me wait. I became friends with several of them and I enjoyed every activity there was. Paula, the activities director is amazing. She has so many activities going on you’re never bored!!!Some of the guests many of the guests , we bonded. Little time in my room I was always out being a Social butterfly. All of the crew the nurses the CNA‘s the physical therapists the charge nurse every one of them I cannot say enough about Maria LG, Maria S, Antonette, Alex, Mini, Michelle, Sarah, Nadia, Ro(kitchen manager whom I’ve known for several years that was such a very nice reunion there) Rosie, Autumn, the PT team and occupational team Boone, Sarah, Mike, Kameron and I know I’m forgetting many names and I am so sorry because each and every one of you hold a place in my heart. I miss you all. I was so sad to leave. I truly had a wonderful experience there and I would recommend this place in a heartbeat. Thank you for all the love and support. I had a lot of smiles there. We had a lot of fun. You guys are great. I cannot say enough positive things and your rating should be 10 stars. Again thank you so much for the wonderful experience.

I spent 21 days here recovering from surgery for a double quad separation from my knees. The nursing and Physical therapy staff were exceptionl! They got me on my feet and walking again, when some days I didn’t see it happening. If I ever need rehab again I wouldn’t hesitate to go here! THANK YOU ALL! Bill

The nurses and CNA’s are awesome. We had a lot of laughs. It may just be because I’m an absolute goofball.

Minnie, Sara G, Maria, Leon, Jennitte. Are so wonderful and amazing. They are so respectful to my mom , and make her laugh. And when she calls for help they are there right away. And sometimes they peek in to she if she needsanything when doesn’t call. And if they are busy the peek to let her know they will be back. And the Evening shift, Antonette, Alex. Thank so much for all you do.

Wonderful. I have had five years post recovery and have not fallen yet. Physical Therapy team did me very well. BLT: No lifting bending or twisting. It works. Thank you thank you thank you. Three flights of stairs. Daily. I live. And the friends I had to abandon there.

Time to your room is short, the staff is great, the food is good although my finicky partner had me go to subway a couple of times. If you have a complaint they are in your room in no time and they fix what bothers you. My partner needed a lot of physical therapy and Mike got him up and going in the time we had there. Linda, one of the rn’s, was delightful. And I can’t say enough about the cna’s. They conduct a weekly care team meeting where all the caregivers meet with us and discuss progress of rehab and determine the discharge date. About a 20minute drive from western Beaverton in forest grove, I highly recommend this place. My experience there has been pleasant.

I’m glad to be able to fill out this form. I have been at this facility 2 × 4 rehab. And I have to tell you, it is amazing. The staff here at Forrest Grove, acute has brought me back twice. I cannot be happier. The staff is incredible. Whether it’s an RNA therapist, a CNA or kitchen help or housekeeping, they all work like the team together, they are absolutely wonderful to the patients. I was treated like a king, and I was so happy to make the progress I made. I would give this 5 stars at the drop of a hat.Thank you for everything you did for me.You extended my life greatly.Thank you

You’d be hard pressed to find a place more warm and welcoming. You can tell every staff member is truly passionate about what they do. If you want the best and most attentive care around, this is your place to be. Shout out to Amanda specifically for being an absolute joy. I would trust them with any of my loved ones ♥️

WORST PLACE EVER!!!!! NURSES HAVE YELLED AT MY GRANDMOTHER! All her belongings were stolen. The doctor gave her medication she didn’t need that hurt her. STAFF WAS NEVER EVER ON THE SAME PAGE.

I’m currently here and it’s been pretty awesome! (I’m writing this of my own free will, the only influence is appreciation) I have some mental issues so that’s probably part of it but sometimes I feel like a queen here! Literally everyone is friendly or atleast all the staff, which is what’s really important. Therapy here is fun but focused which honestly makes it easier to do and look forward to. I could go on and on n on! Seriously! Everyone talks about healthcare food being bland but here it’s delicious. Consistently my favorite parts of the day are when food comes! I think last month we had chili so good I started singing Rihanna’s only girl in the world cause it tasted so good, I’m a Taurus so food, especially good food = happy bull. They are also really cool with visitors, my mom has practically visited every night and no one has been weird or chastised either of us. I really appreciate that because I’m a total momma’s kid. Anyway great place a sincere thank you to all the staff! I don’t see myself forgetting their kindness for the rest of my life. No lie.

Forest Grove Post Acute easily has the best food I have ever experienced in the 10 years I have been working in skilled nursing care as an SLP. It’s always hot, fresh and a widely varied menu. Highly highly highly recommended on this alone.

Dropped in and staff were helpful, worked as a team and respectful of their clients! Clean!!! Staff had the patients out doing their exercises!

An amazing and caring facility. Top notch across the board. Highly recommend for any family members and friends admitting at Forest Grove.
